To begin the process for any new patient, we will need them to fill out a New Patient Information form that covers their current condition, medical history, insurance information and any other information we need to know. Patients can fill the form out at home and bring a copy to the appointment. We will then go through a consultation with the patient to fully understand the patient’s condition. We will also take any other concerns the patient has into account. We will then customize the treatment plan to the patient’s needs. While we treat a variety of foot and ankle health problems, we also provide follow-up appointments to ensure that the treatment is going smoothly.
